Heart failure: how a team approach can optimise patient care- ZOOM WEBINAR

We would like to invite your members to a webinar on heart failure hosted by NPS MedicineWise, in collaboration with the Heart Foundation, as part of the national heart failure program.

We know that a multidisciplinary approach to support people living with heart failure improves outcomes and quality of life. You will hear from a GP, heart failure nurse practitioner, pharmacist and a person living with heart failure about how to improve care. During the webinar, panel members will use case studies to discuss strategies for heart failure management, including initiation and up-titration of heart failure medicines, the role of home-medicines reviews, and patient action plans.
